The origin of endothermic homeothermy and of high metabolic rate in mammals is currently believed to be the result of early (Mesozoic) selection in advanced cynodont therapsids and/or early mammals for either (1) enhanced thermoregulatory capacity or (2) increased powers of endurance and stamina. Selective factors underlying the origin of specialized respiration/ventilation-support systems in mammals are possible indices of the validity of these two hypotheses. One such support structure is the diaphragm, a specialized muscle that facilitates lung ventilation. We tested capacity for maintenance of resting metabolic rate, thermoregulation, and for extended, intense exercise in laboratory rats (Rattus rattus) in which diaphragm function had been completely ablated. The results were virtual elimination of aeroboic scope (active metabolic rate — resting metabolic rate) but resting metabolic rate was unaffected. Thermoregulatory capacity was unimpaired to at least 8° below lower critical temperature. These and other data suggest that the origin of the mammalian diaphragm, as well as mammalian metabolic rates, may have been related to selection for greater levels of sustainable activity rather than for functions associated with thermoregulation.

Objective
In 1990, Fontan, Kirklin, and colleagues published equations for survival after the so-called “Perfect Fontan” operation. After 1988, we evolved a protocol using an internal or external polytetraflouroethylene tube of 16 to 19 millimetres diameter placed from the inferior caval vein to either the right or left pulmonary artery along with a bidirectional cava-pulmonary connection. The objective of this study was to test the hypothesis that a “perfect” outcome is routinely achievable in the current era when using a standardized surgical procedure.
MethodsBetween 1 January, 1988, and 12 December, 2005, 112 patients underwent the Fontan procedure using an internal or external polytetraflouroethylene tube plus a bidirectional cava-pulmonary connection, the latter usually having been constructed as a previous procedure. This constituted 45% of our overall experience in constructing the Fontan circulation between 1988 and 1996, and 96% of the experience between 1996 and 2005. Among all surviving patients, the median follow-up was 7.3 years. We calculated the expected survival for an optimal candidate, given from the initial equations, and compared this to our entire experience in constructing the Fontan circulation.
ResultsAn internal tube was utilized in 61 patients, 97% of whom were operated prior to 1998, and an external tube in 51 patients, the latter accounting for 95% of all operations since 1999. At 1, 5, 10 and 15 years, survival of the entire cohort receiving polytetraflouroethylene tubes is superimposable on the curve calculated for a “perfect” outcome. Freedom from replacement or revision of the tube was 97% at 10 years.
ConclusionUsing a standardized operative procedure, combining a bidirectional cavopulmonary connection with a polytetraflouroethylene tube placed from the inferior caval vein to the pulmonary arteries for nearly all patients with functionally univentricular hearts, early and late survival within the “perfect” outcome as predicted by the initial equations of Fontan and Kirklin is routinely achievable in the current era. The need for late revision or replacement of the tube is rare.

Environmental temperature varies in time and space on timescales ranging from a few hours to long-term climate change. Organisms are therefore continually challenged to regulate and maintain functional capacities as their thermal environment changes. This volume brings together many of the leading workers in thermal biology, with backgrounds spanning the disciplines of molecular biology, cell biology, physiology, zoology, ecology and evolutionary biology, to discuss the responses of a wide range of species to temperature change at all scales of organization, ranging through the molecular, cellular, organismal, population and ecosystem levels. The volume provides an important and comprehensive contribution to the study of temperature adaptation, which, given the concern about global climate change, will provide much to interest a wide range of biologists.

Summary
Studies of thermoregulation and thermal effects have been one of the most prolific and enduring topics in functional biology for the past century (for reviews, see Hochachka & Somero, 1984; Prosser, 1986; Cossins & Bowler, 1987). The popularity of thermal biology stems from its great significance for biological systems. Temperature has pervasive effects on biological rate processes: rapid changes in body temperature alter nearly all physiological functions by approximately 6–10% per degree Celsius over a broad thermal range. Organismal and population-level traits that depend on those processes, such as energy utilisation, growth and reproduction, are also therefore greatly affected by temperature change. Thus virtually everything that an organism does is influenced by and dependent on its thermal condition.
Despite the major impact of temperature on functional capacities, biological systems have evolved in and adapted to almost every thermal environment on earth. Adult metazoans have successfully colonised environments ranging from –70 °C to 50 °C, while the thermal tolerance of dormant stages and prokaryotes is even greater. Individual organisms and their offspring usually encounter a range of temperatures during their life-cycle, varying from daily cycles to seasonal or longer-term climate change. Responses to a given temperature or temperature change can vary markedly between species and between different life-history stages of the same species. Much of the literature of thermal biology has been concerned with determining thermal tolerances and describing patterns of function at different temperatures.
